JT6 Markham Adult Participation Agreement

I acknowledge that participating in martial arts classes, training and competition is a potentially dangerous activity. The possibility of bumps, bruises, scrapes, soreness, sprains twists, related injuries or even death is possible. I could be exposed to illness such as novel coronavirus or other illnesses and disease. I completely absolve JT6 Studio/Ikiro Group Inc./NPR Sports Group Inc./JT6 Markham and any party acting on their behalf of responsibility for any injury or illness I may sustain during any activities both inside and outside of the JT6 facility. I acknowledge that physical contact with my body is both part of practicing and learning this martial art. The chest, buttocks, groin, or any part of the body may be contacted by any part of the training partner’s body during training of a specific technique or incidentally contacted while performing a martial arts technique targeting another portion of the body. I understand that instructors may adjust or manipulate any part of my body to assist in the proper application and understanding of a technique. I understand that I have the right to withdraw from any exercise or drill for any reason and at any time. If the conduct of an instructor or training partner appears inappropriate, the student should inform a JT6 Instructor or director privately. If the conduct of an instructor or any training partner appears criminal, then a director should be informed and the authorities may be notified either by the student or the director, or both. I agree not to bring any claim or suit against JT6 Studio/Ikiro Group Inc./NPR Sports Group Inc./JT6 Markham directors, instructors, staff, guests, students, landlord, or any other parties for any injury or harm sustained by any event short of a criminal act, and then only the criminal shall be the subject of such a claim. I further agree that I will not cause to be brought, nor encourage a claim or suit. I also agree not to cooperate in the bringing of such a suit or claim except insofar as I may be legally required to do so. Finally, I shall indemnify JT6 Studio/Ikiro Group Inc./NPR Sports Group Inc./JT6 Markham, instructors, staff, guests, students, and any and all additional defendants covered by this agreement for all judgments, costs, attorney fees and other expenses incurred as a result of a breach of this agreement. If any clause, sentence, phrase or statement in this document is found unenforceable or invalid by any Court of law, the remainder of this document shall remain valid enforceable and the invalid clause, sentence, phrase or statement shall be considered struck from the document. I understand that if my behaviour endangers other students’ or instructors’ safety I may be asked to leave. No refunds will be given for single classes, day passes, monthly, semi-annual, or yearly membership. All pre-paid 6 months /12 months are not refundable but can be put on hold/frozen with an accepted valid written request and confirmation. For personal, injuries or medical reasons, please send a valid written request to markham@jt6.ca. Please take note that JT6 Studio/Ikiro Group Inc./NPR Sports Group Inc./JT6 Markham has also fixed expenses and fees.

I hereby authorize any images audio or video footage taken of myself, in whole or in part, individually or in conjunction with other images and video footage, to be taken and used by JT6 Studio/Ikiro Group Inc./NPR Sports Group Inc./JT6 Markham on our Website, Social Media, Facebook, Instagram and / or any other official channels, and to be used for media purposes including promotional presentations and marketing campaigns. I also authorize any media material created by myself within the JT6 facility. I waive rights to privacy and compensation, which I may have in connection with such use of my name and likeness, including rights to be written copy that may be created in connection with video production, editing and promotion therewith. The JT6 Studio/Ikiro Group Inc./NPR Sports Group Inc./JT6 Markham and its landlord, directors, instructors, coaches, and students are not responsible for lost or stolen items. Please make an effort to bring all clothing, mouth guards, water bottles and other items home with you. I understand the risks of being exposed to virus, sickness and diseases and I completely absolve JT6 Studio/Ikiro Group Inc./NPR Sports Group Inc./JT6 Markham and any party acting on their behalf of responsibility for any injury or illness or loss I may sustain. I will not come to class if I feel ill and/or if show any of the signs of virus.

JT6 Markham Youth Participation Agreement

I understand and have communicated to my child their right to withdraw from any exercise or drill for any reason and at any time. If the conduct of an instructor or training partner appears inappropriate, the parent/guardian should inform a JT6 director or instructor privately. If the conduct of an instructor or any training partner appears criminal, then a director or instructor should be informed and the authorities may be notified either by the parent/guardian or the director/instructor, or both. I understand that if my child’s behaviour endangers the safety of other children during JT6's activities or if their behaviour is excessively disruptive, they may be suspended from their participation. I agree to pick up my child within a reasonable timeframe, if my child is suspended. No refunds or vouchers will be given in such a case. I agree not to bring any claim or suit against JT6 Studio/Ikiro Group Inc./NPR Sports Group Inc./JT6 Markham directors, instructors, staff, guests, students, landlord, or any other parties on behalf of my child for any injury or harm sustained by any event short of a criminal act, and then only the criminal shall be the subject of such a claim. I waive rights to privacy and compensation, which I may have in connection with such use of my youth’s name and likeness, including rights to be written, copy that may be created in connection with video production, editing and promotion therewith. The JT6 Studio/Ikiro Group Inc./NPR Sports Group Inc./JT6 Markham and its instructors, coaches, and students are not responsible for lost or stolen items.
